NOTICE OF PRIVACY PRACTICES
Life Again Therapy, LLC
Effective Date: October 21, 2026
Your Information. Your Rights. Our Responsibilities.
THIS NOTICE DESCRIBES HOW MEDICAL AND MENTAL HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, HOW YOU CAN ACCESS THIS INFORMATION, AND YOUR RIGHTS REGARDING YOUR HEALTH INFORMATION. PLEASE REVIEW IT CAREFULLY.
Life Again Therapy, LLC is committed to protecting the privacy and confidentiality of your personal and protected health information (“PHI”). This Notice of Privacy Practices explains how your health information may be used and disclosed and describes your rights regarding that information.
Payment
Life Again Therapy currently operates as a private-pay practice and does not bill health insurance companies for therapy services.
We may use limited health information as necessary to process payment for services, maintain billing records, provide receipts, or manage other financial aspects of your care.
If you request documentation for your own records or for possible reimbursement from another party, additional information may be provided only with your authorization or as otherwise permitted by law.
Private-Pay Services
Payment is due according to the financial policies provided to you before services begin. Fees, cancellation policies, and accepted forms of payment will be discussed during the intake process.
If you have health insurance, choosing private-pay services means Life Again Therapy will not submit claims to your insurance plan on your behalf.
You are responsible for understanding whether your insurance plan offers any reimbursement for services received from an out-of-network provider.
Good Faith Estimate
If you do not have health insurance or choose not to use your health insurance to pay for services, you have the right to receive a Good Faith Estimate explaining the expected cost of your care.
Life Again Therapy will provide a Good Faith Estimate as required by federal law.
If you receive a bill that is substantially higher than your Good Faith Estimate, you may have the right to dispute the charge through the federal patient-provider dispute resolution process.
